Throttle control cable adjustment, rough shifting

I've been having problems adjusting my throttle control cable on my 91 rt4wd automatic. I have been adjusting it at the top by the firewall and intake manifold and when I do it either idles too high or still shifts a little rough at times. Should I start adjusting it at the throttle control lever near the tranny first and work my way up? and how much free-play should there be? Also the backing nut up at the top keeps coming loose, is it ok to use thread-lock there or is there a better way to keep it in place? And when I adjust the throttle cable too tightly and idles high, it shifts very smoothly, so im pretty sure that this is the cause of my rough shifting.

I'm sorry, I've never laid eyes on an auto rt. Is there a transmission control cable tied into the throttle? From your description it sounds like when you raise the idle, you're affecting throttle input to the transmission. In my (limited) experience w/ these engines idle speed is set at the throttle body bleed screw , not with the cable.
Sounds like you need a manual for a step-by-step on how to adjust.

Yes there is multiple stages of adjusting the idle on these motors, you cant just adjust the cable and hope it works. You need to adjust the cable, the idle screw and potentially even clean out the throttle body cause they get gummed up with oil deposits.

Someone needs to make a writeup on adjusting the idle and throttle cable!
