Need Help With Tranny

Im trying to figure out if my 4th is out or just needs adjustment i have all my gears but 4th, it just slides into 4th even without disengaging the clutch.. And my fifth grinds a little when putting into 5th.. Idk if it could be something to do with adjusting or its just out.. Can anyone help or give me a couples pointers i would appreciate it. Thanks in advance

    Take out your center console, revealing the linkage.

    I once lost reverse. Took the console out, to discover the linkage had come loose and basically adgusted itslef to the point where it didn't work anymore.
